Population: Adults with histologically documented, unresectable locally advanced or metastatic urothelial carcinoma of the bladder or upper urinary tract, measurable by RECIST 1.1, eligible for first-line enfortumab vedotin plus pembrolizumab and with detectable baseline plasma ctDNA. Patients without progression after six induction cycles are allocated by post-induction ctDNA status; those with ctDNA clearance are randomized to continuous combination treatment or enfortumab vedotin interruption with pembrolizumab maintenance.
